If you have a form that contains a great deal of questions on a single page, you may want to consider using headings – or, in Form Builder parlance, headers – to break up the content of the form.
To add a header to a form:
If you do not have a form open for editing, open it, as described in Opening a form for editing.
Locate the section to which you want the header.
Click Add a header. The system displays a screen with the heading Add a header.
In the Header field, enter a name for the header.
If appropriate, use the Description field to enter any supplementary text that you want to display below the header.
Click Save. The system saves your changes and again displays the Form Builder: Your Form Name screen.
